Need help on a 1911 issue

Pistol is a SAM commander in 9mm, about 1000 rounds through it. Every 5th or 6th mag, the last round extracts but doesn't eject from the frame. The spent round winds up extracted, lying on top of the follower, 180 degrees around, the primer at the chamber end. The round just falls free when the mag drops so it isn't causing malfunctions, I'm just wondering what would cause that.

It only ever happens on the last round, with a variety of mags and the gun has no other feeding issues to speak of. Any ideas?

find mags with a different follower design, and see what happens. from what you write, it only happens on the last round, and never on any other round. and clearly the only thing that "changes" between all other rounds and the last round is that on the last round, the follower comes into play as the last empty shell is being extracted.

if you drop the mag before firing the last round, how does it extract / eject? same weird hangup? or something new altogether?
 
As has been noted, probably an extractor tension issue, but it's strange that it's only on the last round. The magazine is not supposed to be part of the ejection system on a 1911.

Have you tried hand cycling empties in slow-motion (remove the recoil spring) to see what's going on?

Could possibly be an issue with the extractor clocking, or the extractor length being out of spec.
