Too much lube in neck and shoulder area causes this problem.

Yep too much lube on your case neck. Your die may also have built up of lube in the neck area,clean.

Make sure the vent hole in the die is not plugged up either

Take the die apart and clean it removing any excess lube .

The reloading manuals tell you to wipe the shoulder and neck off before sizing. The tapered case causes the excess lube to be forced upward onto the shoulder

And as stated above wipe the excess lube from inside the die. And at the first sign of shoulder denting fix the excess lube problem.
