Could have been using dummy rounds (I hope). Sounds like he was working the action in super slow-mo and then wondering why the cartridge was hanging up. Since he hasn't returned I'm just speculating.
 
Never slowly release the slide, rack it back and LET IT GO. Semi Auto's are not designed for a slow release back into battery. They need force to strip a round from the mag, bring it up into the grasp of the extractor claw then push it up the ramp into battery. It needs force for this to happen. I would go with Chip mags, Wilson mags are too tight with 8 rounds in them and ACT/DLASK feed lips crack.
 
Its like slowly closing the bolt on the sks. The bullet will scrape all the way up the ramp, or get stuck trying to get there. Ya gotta SNAP the sucker !
