On question 16e where it says "During the past 5 year, do you know if you have been reported to the police or social services for violence, threatened or attempted violence, or other conflict in your home or elsewhere?


I had my secondary rights read to me last year due to a guy who tried to accost my friends daughter and lead her down an alley just down the street from where they lived and later that week, I caught him outside they're place of residence. Would that require me to check off yes in question 16e and send in an explanation as requested?

i would definitly answer no, as there probably was no occurance report generated.

If you know about it, then answer "Yes". Otherwise, you don't know, so answer "No".
