I've read the other threads, tried a few tricks, including sending it back for warranty. No luck.

Could be the extractor needs tuning or possibly debris or burrs on the hook or maybe even a worn hook (hardly likely if it is a new pistol). If it's new, it's possible the extractor just needs more tension.

Remove the slide, slip a spent case under the extractor hook and it should stay there. If it slops around or even falls out, the extractor needs some more tension.

Thanks Trinimon, I'll be checking out the Wilson Combat video...here's hoping. The gun has maybe had between 500 - 1000 rounds through it. They come with Evolution Gun Works extractors, and it was dead straight, so I have bent it to increase tension. What I notice is that it only seems to happen on the last round, when it can ride lower, closer to the magazine and gets jammed into the magazine lips.
 
Dirt can build up behind extractor in the hole and a cleaning can fix it .. till it gets dirty again

If after tuning Extractor and it works ok for a while then fails again
replace/ tune a new extractor of a well known company.... there are some extractors too soft to hold tension
 
The extractor tension should also be sufficient to ensure that spent cases come out of the ejection port with no magazine in the gun, as opposed to stovepiping or dropping down the magazine well.

question before you do anything else...

Do you notice ejector or extractor marks on the brass? A slide moving too fast will marr the brass, and close on the case (crimping the front) before it can leave the ejection port.

If you have a heavier recoil spring, give it a try. Did anyone try to lighten the trigger? Done improperly, this can increase slide speed.
 
Since its the last round it's more then likely the mag I'd suggest trying out a different mag first before trying anything else.

If you only load 1 round in the mag and shoot it will it do it?
 
Your extractor probably needs more tension.The reason why it happens with the last round fired is that there are no following rounds in the magazine "supporting" the fired case
and the extractor loses control of the fired case which drops down into the now empty magazine jamming itself into the lips of the mag(often damaging the feed lips) or else gets jammed into the breech face as the slide slams the case forward

A good test for adequate extractor tension and function is that the gun should reliably extract and eject with no magazine in the gun.
