Here it is: 9mm 115g Winchester white box out of a stock sig.
Squeezed the trigger, "BAM", no bullet and lots of smoke from the breech. Gun is fine and ran beautifully after.
What happened?

No flash hole does seem the most likely.
If you popped out the primer with a pick or something you could check the primer pocket. I would guess there wont be a hole.

*edit*
Are the missing parts of the primer what blew out? It almost looks like a primer that was crushed in sideways. If it was solid and round before firing and those are just missing pieces, then I stick with no flash hole in the brass as my guess.

There didn't seem to be any brass on the breech face. Just a big "POOF" of smoke as I squeezed the trigger.

That's what that looks like! I couldn't put my finger on it, so to speak.

How do I report this to Winchester?

I'd do a search for Winchester corporate sales or customer service in the U.S. and e-mail them for instructions to send a pic.

Well good news is you survived.
Now is the time to start loading your own ammo as
mentioned above.
Trust no one butt yerself.........iff'in ya dare.
