quck little bit of help please

i have these forms right an at the end where i solmely declare.....
it has a section for this
"declared before me at" _____ of ____in the privince of_____ this_____day of___ad.20____

is this just some sort of address? is it aslong as my dates are right, just fill in with our town address in aus or what?

and with the Signature for the Commisionar of Oaths? (to the right of the page) under my signature- what goes there???

any help be awesome thanks

Declared before me at the City of Edmonton in the Province of Alberta this 6th day of February A.D. 2020. Just an affidavit. Change the city to town or county as applicable, same with the province, the rest is just a formal way of dating it. Under your signature simply print your name so it corresponds to your signature, full name.

Leave blank until you are with the Commissionar. Should be done in their presence.

Some of the space is for the official stamp.

Probably just a typo but "CommisionAr" I believe should be " Commisioner". It's a very long time since I was one!
