Interesting, I don't see how lubricant could cause a failure (I wonder if it was at the root where the undercut for the threads meets the shoulder of the barrel) - sure looks like it though.

The barrels were overtightened. It was apparently done by a single employee on a single shift who was cutting corners to save time.

Looks like a over tightening bolt failure so the over tightened barrel story makes sense to me.

Can someone explain how catastrophic failure occurs by over tightening a barrel? Is it done by some machine that cranks it so hard cracks develop?
