I have a 308 win 99c owner brought it to me for a cleaning it’s first in 40 years I’m sure, fired cases were sticking not ejecting after I cleaned polished the chamber, removed the bolt cleaned it without total disassembly ejector looks like it’s working no visible damage.
Tried 3 different rounds two ejected the one that stuck on the left looks like firing pin struck twice any idea why it would do this?

I'm not a gunsmith, just a guy who's fired enough rounds in enough different guns to have opinions. Here are three thoughts.

On Browning belt-fed machine guns the chambers were awfully sloppy, but the extractors were more than a little tired. We learned about headspacing and function cycles. So, how well is the extractor engaging? Is its tip burred or the channel sludged? Is the spring giving positive engagement?

The breech face could be scored - from Johnnie Kitchentable gouging away at the rims of stuck cases with his pocket knife. Maybe the breech isn't closing completely for that reason, and the extractor not fitting properly.

I'd also go back to the chamber walls. Was the polishing longitudinal or lateral? Did the last polish leave rows of grippy ridges versus long flutes?

Extraction is when the fired case is mechanically removed from the chamber by the working extractor. Ejection is when the fired case is mechanically ejected from the firearm. Are the fired cases not being extracted from the chamber consistently? Or are they hanging off the ejector, having to be physically removed from the action?

I’ve had this happen before with a savage 99. My prob was that the bars wasn’t clean from lube from when it was reloaded. My advice would be to mic the three pieces of brass and see if there is any difference in dimensions. The one piece on the left could be an indicator of pressure for some reason the gun didn’t like that round. Check brass first then do a chamber cast and go from there

Had a 99 in 300 Savage. Worked super with Remington or Dominion factory loads. Tried Federal factory loads and cases stuck in chamber every time after firing. Obviously dimensional difference just stayed away from Federal. Yours had a problem with multiple brands apparently. An older 99 guy told me to keep chamber dry and free of oil, claimed it made cases stick. Dont know if that holds water or not.

Rather than struck twice, it appears like it may have dragged across a stuck firing pin. The 99 has a firing pin return spring that is easily missed/lost if the firing pin is ever removed. Its purpose is to prevent the nose of the pin from interfering with the case as it slides up the bolt face. Just something to consider.

Are those reloads? None of that brass looks super clean. Several factors can make a 99 fail to extract. Dirty chamber, hot reloads (for Savage 99) , dirty brass improperly sized brass among others. I believe this is an ammo problem not a gun problem.

Reloads or factory rounds?
If reloads, that solves it. If factory then I suggest a smith familiar with the 99.

This is the reason why the cases won't extract. With that return spring lost every so often your firing pin is locked forward into the primer of the case and when the Savage 99 bolt unlocks it has to tilt down. If the firing pin is embedding itself into the primer, the bolt can't tilt to unlock without making that huge gouge.
