If the rifle is under warranty then this serious incident will certainly get the manufacturer's attention.

Yes it did and apparently the operators of the board here don't like this story as they seem to keep moving posts of this serious situation to never, never land.
Under the right circumstances this can happen because of the way the bolt is designed and a novice user. Refer to your Axis user manual Section 3, Page 5 (cocking piece pin)
Always check that your firing pin is never stuck in the forward position for any firearm, even bolt actions!

Do you mean this has happened before or this is a known fault that hasn't been acknowledged ? Or is acceptable?
This seems like a bad design (dangerous) for people to be complacent about
 
There is a couple of other threads on this, basically, the rifle fired out of battery, the casing exploded, according to the pics in the local newspaper, the primer is in position and was struck. The rifle was new and being sighted in, the ammo was new. The user was inexperienced with the rifle. The other thread has a good opinion on the design flaws of the axis that allowed this to happen.

The movement of the thread is likely because of the flame fest that I got when I mentioned it was a Savage Axis. I was rather shocked by the level of hate directed at me for mentioning the make and model. Fortunately that has all been deleted.

The link to the other thread didn't work for me . I for one can't understand this flame fest talk . Doesn't everyone want to know about these hyped up cheap guns being defective or dangerous . And if people blame the victim for not knowing his gun , I would expect every gun to have engineering safeguards so these things can't happen or they should be recalled
 
The guy tried to close the bolt three times and it would not close so he slammed it hard.most likely hit the trigger well hammering on the bolt. If it feels wrong the first time you try to load a round stop and look.

yea, he said this was 'unavoidable' or some such term. I think otherwise.
