SKS failure to feed

My son took out his 1954 SKS (non refurb premium weimajack) the other day and it would fail to feed on the 4th round every time. He did say that it could have been the third round but it was always consistent. Last round feeds no problem. I wish he would have paid closer attention.
The round would jam up on the loading ramp and shove the bullet into the casing every time.
This rifle was stripped and cleaned upon purchase and I am going to do that again to see if it helps. About 40 rounds total so far using surplus Russian ammo. The rifle is otherwise an immaculate example.
Any ideas?

Likely a mag pinning issue. This happens when he follower is not able to fully pivet.

These would be my first guesses, make sure the spring is in right and make sure the mag follower is able to fully pivot, if it can't pivot to match the angle as it moves it can cause feeding issues.
 
The spring is installed correctly.
I need to have a closer look at the pin welded to the follower. The mag on this particular rifle is pinned by adding a spacer to the follower. Maybe this piece is hanging up somewhere.
The boy has left with the keys to his cabinet so I guess it will be tomorrow night.
 
Finally got a look at this rifle.
The follower is indeed bound up because the rivet holding the pin is a bit tight. Some lube and wiggling improved it a bunch but I think it could be even better so I will revist this next weekend. Not a problem with how the rifle was made, just a problem on how it was limited to 5 rounds.
I'm 100% sure this is the problem. Thanks for all the tips.
 
I took my magazine with me to a friend's house to watch a hockey game. I basically worked it for the entire game. At the end, it was very smooth... and no jamming. The Leafs lost. :)
