It's used to happen with the Sig P226R all the time. Why??... My fat thumb was hitting the slide stop. This is less of a problem with the Legion series as the slide stop does not protrude as much. However, now that it doesn't protrude as much it's harder to lock the slide open... It's a hard life.

Common problem with a Sig 226. All my others are not a problem. The slide release is very close to the thumb. Under recoil the thumb bumps the slide release and voila no holdback. I figure it comes from the shooting position I use for most of my other handguns with the thumb high on the safety. Muscle memory can be good but is hard to overcome.
