Smith Mount mounting instructions needed

I have bought a second hand but unused Smith Enterprises mount however the mounting instructions did not come with it. I have been told that Smith Enterprises have them on their web site but have looked and don't see them.

Could someone please either post the instructions or mail me a copy. I'll provide my address and cover postage if that's the best method.
