Well, it isn't symetrical front to back and it has rounded corners on only one end of the rail mount section so it would appear to have a very distinctive "front".
There is an unthreaded hole in the floor of the rail mount section, but no cross bolt (a la picatinny) so I'd assume a plunger goes in that hole to provide the clamping force onto the rail.
That brings us down to the bottom section which has mounting holes. I would have thought that this is where you'd mount the eccentric cam to work the plunger, but then there wouldn't be any attachment points so I'm a little confused.
Finally, we have the ground down or flattened ears as the only real clue to the devices actual purpose. To me this would indicate a landing spot for a spring loaded flip-up or flip-down device. With the orientation of the drawing, a flip-down vert grip would be one option, but there doesnt appear to be any built in stop to prevent over-extending the grip so I'll assume you posted the drawing inverted to throw us off and I'll guess a flip-up front site.
I probably way over-thought this and am completely wrong....
