When I had this problem with my Gen1 tavor the issue occurred with one round left in the mag. On certain mags the follower would push up the bolt hold open just enough to get clipped by the bolt with one round left. I remember LAR mags being a problem and some pmags.
IWI 5/30 mags were fine obviously as well as the AA beowulfs.
