When a cylindrical vessel is internally pressurized, stress in the radial direction (hoop stress) is much greater than axial stress. About 2.5 times as great. This means that unless the vessel is much weaker at the ends, it will always fail radially, by expanding outward. Look at any kaboom of a firearm that is known to have been caused by overpressure and you will see that the chamber and barrel are bulged.

Here the chamber and case are not bulged, this case failed at the end. Therefore the case had to have been weak at the end. Was it pre-cracked or loaded too many times? I suppose it's possible, but I don't know for sure. But it is clear that it was a case failure, and had nothing to do with primers or double charges or bullet setback causing pressure spikes.

Whether a double charge or not, clearly the pressure was too high. Was it a 'CZ' load (very short OAL)? Combined with an unsupported chamber that's what happens.

The case failed where it was not supported by the chamber. Feed ramp area. Because the case failed where it was unsupported, the chamber was not bulged. The unsupported portion of the case is much weaker than the chamber, so the case blew, and the chamber was not damaged.
 
been loaded too many times- had the same thing happen myself, and i use a progressive, so there is NO chance of a double charge- lost the little knob on the slide stop that holds the slide back as well as blowing the mag all to heck and gone - THIS IS THE CHANCE YOU TAKE WHEN YOU USE RANGE BRASS
Bull$hit. You can certainly doublecharge with a progressive press.
One example is as you are raising the handle (shellplate in the up position coming back down) the shellplate jams because the decapper didn't push the old primer out all the way. You then push back down on the handle to unstick the primer and voila a double charge.
There are many other ways to do it.

If a doublecharge was not possible RCBS wouldn't have trouble keeping lockout dies in stock.

I also know someone who blew up a $5,000.00 SVI on a Dillon 1050 with a doublecharge. Everyone in the room knew what had just happened.

If you look at how flattened the base of the case is it's a sure sign of overpressure. The unsupported part of the case blew out since it is the weakest part.
I have no idea if you have ever looked at a 9mm 1911 barrel in person but there is quite a bit more material there than any other type of 9mm barrel on the market.

B.S, split/blown cases can happen with spanky new brass as well...
