60$, make sure you fill out the form right. Make sure to stamp the little paper with your name, guaranteur's name and signature in the back of the photo. I can't find the address at the moment but by the time you get the form all filled up I should have it for you. Congrats btw
Getting a PAL is a huge waiting game. It will take a while, start saving up your money now so once it comes in you will be able to treat yourself

I think they changed that a year ago, its 60$ for a PAL, once you have a PAL the RPAL is no charge.
I just got my RPAL a month ago. It's still $80 for the RPAL says it right on the form.

It's $80 if applying for both at once, no charge for upgrading a PAL to an RPAL at a later date.
