Cleaned barrel for each first shot? Guard screws properly tightened? All scope mount screws properly tightened? Otherwise I would look at action bedding allowing movement of the rifle in the stock from handling between groups causing wood to metal pressure changes. You might consider trying a different powder as IMR 4831 is one of the more temp. sensitive powders. If it does it with another less temp. sensitive powder I think I would go directly to bedding improvement.
If if only happened the first time I would say a clean cold lightly oiled barrel. But since it happened again with what I would assume was only a cold barrel, I would look at the bedding screws then the bedding itself.
