What's wrong with my NEA15 BCG?

It will hold up the BCG for every 5-10 rounds, I wrote email to NEA and they said it's the mag issue, so I changed to the mag purchased from NEA, the issue still there.

What's wrong with it? Is the problem from the buffer tube?

Your bolt hold open appears to be engaging during firing since all you did was drop the bolt to continue. You appear to have an aftermarket bolt hold open device. That is likely the problem. Recoil is engaging it or you are bumping it with your finger.

I have a Magpul b.a.d installed, is that cause the issue?
 
It could even been the spurs of metal when buying a new gun, you might want to add a lot of CLP and continue to break it in..
 
I had a BAD lever on my AR that would cause FTF's, and wouldn't hold the bolt open on the last round. I removed it and no more issues.

Not sure how it would cause FTF's though, either way I just removed it and didn't look back.
