winchester defender 1200 (i think?)

I'm having problems with my shotgun. When loaded up with 7 rounds it will feed them up until the last shot which gets stuck in the mag tube or even pops out the bottom when you rack the pump. Hope that makes sense. Does anyone have any ideas what that could be and a possible solution.
 
Good questions, not my area of expertise but I took the spring out and it seems okay and the follower is sliding smoothly. I'll be taeking it apart again later in the week and double checking everything again. Thanks for input.
