It happens sometimes with my TAR21 as well.
Yeah, it's very common for the brass to get a good ding on it on the way out. The extraction is very reliable (violent) and I'm pretty sure it gets slammed against the brass deflector with some force on the way out.
Its VERY easy to accidentally hold the bold catch down while shooting from a benched position, so that it wont engage. Also, "Not really" riding it won't cut it with the Tavors, you have to make sure you don't ride the charging handle at all. Getting a stronger recoil spring mitigates this somewhat.

The dent on the brass actually comes from the locking lug on the right side of the barrel lugs (The star shape thing on an AR15, but Tavor has like 3 lugs). The ejector(springy rod) is so strong that it'll push it to the right onto the locking lug as it gets ripped out of the chamber area.
