I took an inspection light and looked down the bore, cylinder and barrel line up cleanly. Wondering if perhaps you're looking at yours from a slightly slanted angle?
Can't find my digital calipers, but took a .45 cal musket ball that would not fit in the muzzle and it rolled down the breech about 1/3 through the barrel before stopping. Perhaps it's just a long forcing cone. My Snider-Enfield has a tapered bore so i'm not particularly concerned if this revolver has one, neat more than anything.
I cleaned and inspected mine in the morning, and then I took it to the Range yesterday afternoon.
Unfortunately, Canadian Tire was the only place in town who had 45 Colt available yesterday. So, I bought 2 x boxes (40 rounds) of Federal HammerDown 250 gr for $120 + tax = Ouch! (Federal is offering a 15% rebate - so I get a $24 refund).
That HammerDown ammo is first class. I had no issues inserting or extracting rounds. Surprising soft recoil for a 250 gr - 1025 ft/Sec round.
-I brought my 12 ga Blueline pump with me to get my recoil fix....
Anyways, the revolver performed flawlessly for me. I only shot 22 rounds of the 45, because the novelty wore off fairly quickly at $3+ per trigger pull
I also put 15 rounds of 000 buckshot through it. But stopped because I got a spread of +4' from a distance of only 5 meters. 000 buck is useless in this gun - unless you're at very, very close range. Birdshot would be even worse.
IMO, this is a 45 Colt revolver posing as a .410 shotgun, and I'll be sticking to the 45 Colt.
-I've ordered 14 lbs of 45 Colt from SFRC @ $1.47 round (delivered). It's heavy Ammo.
I agree that the double-action effort is way, way too hard. I only shot it SA from the shoulder.
Accuracy was very good at 7-10 meters. I don't like the pop-up sights for both eyes open shooting.
I've ordered a laser for the bottom rail, and I'm researching a 4-6 MOA red dot for the top.
I inspected it when I got home, and there's no evidence of a misaligned bore/cylinder on mine.

A few people have now said these revolvers that shoot 45 colt/acp don’t shoot shotgun shells very good which makes sense
If one is to feed these a steady supply of 45 colt you need to reload your own
You can make cheap ish ammo using cam pro or cast bullets

its one cylinder that looks rough cleaned everything even under ejector wonder don’t think it’s ammo , it one cylinder, maybe time to ask TI

Tried some more 45 colt and some 410 slugs. Slugs are comically bad, missed all shots on a 5 yard man sized target that my smoothbore XR410 could easily hit at 15. 45 colt was decent' all shots on target, had a few sticky rounds, seemed to be slightly out of spec ammo rather than individual chambers on the cylinder. Found that the sticky cylinder issue from earlier can be resolved by applying sideways pressure to the cylinder as you push the latch, pops open. That said it's definitely stickier than my XR410 when doing side by side comparison, even allowing for the older gun being much more worn in.
