I've seen them used in a few movies, like No Country for Old Men. but how realisitic are they? do they actually exist?

Yes...

Suppressed .410 shotguns have been used in Great Britain for rat and varmint control for a number of years. I recall seeing pictures of some but they don't resemble that picture above at all. They look like a small .410 shotgun frame with a 10 gauge barrel attached.

his shotgun was semi auto, low powered loads would be a no go, i think its a mossberg 930 ? i may be wrong.

the low level of sound is unrealistic for sure

what they usually do is have a 12g pipe, ported like its going out of fashion, into the chambers of the suppressor. that way the shot comes out the end like it would out of an non choked barrel

considering the shot is out of focus there's no way of knowing whats the exact size of that hole.

Laporte marked a subsonic supressed over and under for skeet shooting. I saw one in NZ, but didn't get a chance to shoot it. It wasn't silent, but it made a funny sound.

shotgun suppressors do exist

but you have to use sub-sonic shells to have the desired "pooofftt" effect..otherwise you will still here the crack when the slug breaks the sound barrier.

yeap, functions the same as a suppressor on a rifle
supersonic = crack
subsonic = pop

I read somewhere that the loads have to be way under subsonic for the silencer to do anything - I think it was an article from Gunwriters on the Web.

I did get to do some good driven pheasant shooting as well as night ducks flighting into a pond under the full moon. Unfortunately I did not have the chance to fire a suppressed shotgun.
