I didn't realize they even came with an extention initially and don't recall ever seeing one come to think of it.You'll notice that the new Remington 870 Magpul edition no longer has a magazine extension on it.
They changed the location of the lug-ring on the barrel. So the magazine tube is all one continuous piece.
Same design as a Mossberg 590 actually.

Tighten the nut loosen the extension tube slightly.
I wonder if your problem isn't a shell stop that has goop behind it and can't move properly. This can result in intermittent cycling issues that might be blamed on the magazine. Scrub the shell stops on both sides using lots of solvent, then if possible, blow them out with compressed air, and wipe dry. You might even want to flush them with water, then blow any remaining moisture out with compressed air. I'm not crazy about shell stops that are staked in because of the maintenance difficulties they present, but the 870 has always been built that way.
The whole thing screams a QC issue with the extension.
The 'On occasion' part is the important part. Worked ok for a while with the snap caps, then quit? It happen with all live ammo too? You know anybody with a Choate or other aftermarket extension? The whole thing screams a QC issue with the extension. Assuming the thing feeds properly without the extension.
The thing still under warrantee? You get two years. You may want to contact Remington.
