While installing rings with the Offset Inserts, I started at 15 inch pounds and rotated from screw to screw. I went to 20 inch pounds and the four screws were torqued.
Three screws on the rings were then torqued to 22 inch pounds but one side would not.
Suddenly there was a “pop” and now there was a gap between the upper and lower rings on that side.
There appeared to be a piece of metal that had broken through.
The rings were removed and a photo taken. The picture is as close as the camera would function but I do believe you can see the problem.
Only one of the three screws will screw back out as shown on the other half of the upper ring in the photo.

Anyone have experienced this problem before? Have sent the info to Burris Customer Service.
