This is what rings look like:
For further confirmation, rub some oil on the outside of the barrel to make it shiny. The look up into a light and scan down the outside of the barrel. At this point, you should see the bulge if there's any. Also, pass a dry snug patch through the barrel slowly and feel for any change in resistance.

Was it Federal brass? Federal brass in .40 with a headstamp "FC" is known to be weak at the web. There are instances of factory ammo marked this way blowing out at the web. They corrected this and now headstamp it "FEDERAL ".

I've had a couple issues with Wolf ammo in the past before I started reloading pistol ammo and have stopped using them since. In the cases I've had and also my buddy, the bullet got pushed into the case during feeding. Fortunately they caused a jam and had to be cleared. That could have been a nasty kaboom incident. Could be what happened to the OP.

That looks like a normal case head separation which is normally from just brass past its service life if it were aeither a double charge or light load it most likely would have Kaboomed. Also don't shoot other people's reloads it's asking for bad stuff to happen.
Is Wolf not licenced to produce re-manufacture ammo? I thought there were controls and processes in place to make sure re-manufactured is compliant, safe and legal?
And Ya, I don't shoot other peoples reloads and I don't share my reloads with other people.

I've shot the firearms outlet Canada 9mm reloads with 0 issues in about 600 rounds now. Normally I shoot American eagle, but I decided to give their stuff a try because it was really well priced. I bought another 1000 rounds the weekend after my course!
Here's a pic of 5rnds of 45ACP that the bullet got pushed into the case. You could actually push the bullets in by hand...
We had a box of Wartak 9mm that was the same. The bullets were not crimped well enough and you could push them into the case by hand.
I'm not going to knock any product here, but I've had experiences both ways with licenced remanufacture ammo. While about a couple thousand rounds each is not much, I will say I've had great results with WASP Munitions.

Is it possible that the KKM barrel is undercut at the feed ramp excessively?
The casing seems to be cracked and blown out more at the bottom.
Also, I seem to recall reading where Glock was experiencing this with some .40 caliber barrels when they undercut the feed ramp more, to facilitate even greater feeding reliability.
I have also heard of instances where bullets telescoped inward, creating very high pressure on firing.(not barrel-related)
Standard .40 cal. ammunition runs at 35,000 PSI, which is as high as .44 Magnum.
Telescoping inward by 1/10" reportedly doubles the chamber pressure.
Great results in what sense? Reliability? Sure they are reliable. However, in the accuracy department they are rather mediocre. Some time ago I purchased 1,000 9mm Luger WASP re-loads and they do not measure up in accuracy in my three 9 mm Luger cal pistols to the PMC Bronze or the Blazer Brass brand ammunition. Since all three quite diverse pistols do not like them that tells me that they are not accurate rounds.
