Holes in empty cartridge?

Any chance there is a scope mount screw hole in the receiver that has been drilled right through to the chamber?

Does this happen every time? Can you tell if it’s on the top of the shell? Can you probe around in the chamber with a tooth pick or a little piece of wire to see if there’s a void in there? Can you post a pick of the chamber and action?

It sure looks like a sight was mounted, and the hole was drilled to deep (either right through, or almost right through, and it finally gave way.) and the brass is blowing out into the hole. If that’s the case, it’d probably be super hard to extract...
 
I did it ! Thanks for the help. I called my brother to ask him what he did . He told me that it was a different action at the beginning... Probably a hole from a scope mount. Thanks guys.

It extracted fine but and shoots ok but when i looked at the empty ones i was concern.
