Hungry and I spun a .223 LTR barrel onto a 700 action this summer, and it headspaced perfectly without any adjustment, so it does happen. The flutes weren't oriented exactly right, but we considered this to be inconsequential. The rifle shoots really well.
It is my understanding that Remingtom makes boltheads in three sizes, coded on the left lug - plain, a dot, a groove. The bolts are selectively installed. Maybe this is why it is possible to achieve correct headspace occasionally without adjustment. But I have never seen a barrel index with markings or sights absolutely correct without adjusting index.
