M12's sporting Poly Chokes were all after market. Older Polys were silver soldered on, new ones are threaded. Poly Choke is steel compatible but the older models may shoot off if larger steel pellets are passed through them. also, steel should never be fired with a constriction tighter than modified. Further, if you remove your Poly Choke you will be stuck with a cylinder bore short barrel as barrels were cut off to accommodate the choke device. I would recommend you pattern your shotgun as I have one with a poly choke and it prints spectacular patterns in around the modified setting.
Yes the unit was but they could also be ordered when you purchased the gun and factory installed by winchester just like they did with the cutts
Normally no choke barrel markings on the barrel and an alternate adjustable choke fitted is a factory installation
Up until a year ago they still produced the original style as version 1 and the new threaded known as version 2
Unfortunately I remember reading they went bankrupt since but that has not been confirmed

I have checked the Bankruptcy Court for the ED of Wisconsin and while I have found a few instances of a Todd Martin being in the system, there is nothing for either Poly-Choke or Nitram LLC. I tried to check the Wisconsin corporations division to see if the company is still active but the site is down for me at the moment.
Had Poly-choke or Nitram gone into bankruptcy, the trustee would be obliged to return 3rd party property to its rightful owners. All you would need to to is send a letter to the trustee with the identifying features of your property and request its return. However, there is no bankruptcy filing for either business entity so that is not an option.
I also checked the ATF and as of July 2017, Poly-CHoke is still listed as holding an FFL.
I suggest you send a letter of complaint to the Wisconsin Attorney General Consumer Fraud Divsion and complain that Poly-Choke is taking money from out of State consumers but not doing any work and not returning property that has been sent to it. You have made several good faith efforts to contact the business but never get any answer.
You might also complain to the ATF that one of its licensees is taking gun smithing work but not returning gun parts. This could be indicative of bigger problems at the licensee that merit the ATF giving it a good look-see.
