would the bolt and extractor not hold the primer in with the case?
ahh get it... streched case bounces of ground primer falls out...Until the case is extracted
mixed brass could have picked up somebodys old stuff and reloaded it, i have reloaded mine 3 or 4 timesHow many times was the brass casing reloaded? I suggest you check the rest of your lot of brass for excessively worn or out of spec' primer pockets.

I've had reloads lose their primers on the way to the range! I give them all a light tap at an angle on the rim now. Primer loss can be a sign of overpressure and a good indicator that the brass is done. Sure you can peen the pocket back into shape enough to JUST hold a primer in, but you risk the ignition gasses leaking past the primer and torch-cutting your boltface. Cheaper to toss the worn case and start fresh.
