Factory loads have caused problems as well. Machines make those factory loads and we make the machines so mistakes will happen.
There was a time I would have thought you were nuts saying this but not after seeing my barrel frost up and a choke tube splitting when hunting out your way. It gets cold here but a totally different cold if that makes any sense. So yes watch for frost build up. take care

How in gods name do you get twice the powder, a wad, and twice the shot into a 2-3/4" hull and not notice. I can't put a double load of Clays underneath 7/8 of an ounce without exerting a stupid amount of pressure to get a crap crimp that has an 1/8" worth of crushed wad fingers sticking out of the crimp. And yes, that was just to test if it was possible... it can't matter it you're using a PW or a Hornady; there's only so much room in a hull!
I agree.I use 2 Ponsness 800b machines and you would know there was an overload by the shell not crimping correctly and double powder would show double shot too.
My thoughts are this gun probably had a wad blockage or a hull separation blockage where part of the hull may have remained in the barrel (I've seen this with Federal Gold Medal plastic hulls and have heard of the inner base wad of older 2 piece winchester AA hulls separating and lodging in the barrel).
It may also have been a case of loading the wrong powder by accident.....
It would be great if someone knows what happened and could fill us in. It could prevent a similar accident from happening to any of us who reload or shoot clay targets.
Then there'd be absolutely no explosion. Without a wad, there's no containment for the expanding gases, and the pressure just escaped uninhibited from the giant hole in the end of your barrel. Furthermore, without the wad seated properly on top of the powder column, the powder won't ignite uniformly. You'll get one hell of a fireball, and a very dirty barrel as it blows still-igniting powder down the bore, but not much of anything else.

Occam's Razor + where the barrel burst = Obstruction in the form of a separated base wad that didn't get noticed. Of course, that's just my guess.

Agree on your comments to the no wad. You beat me to a response, other than I would have added if you could fit it all in how would you miss trying to close that shell and by some fluke you did you would have to be brain dead not to notice the weight of it when you put it in your barrel to shoot 2 -2 1/4oz of lead I notice 1 1/8 from 1 oz when shooting target loads.

Beyond the illogical mechanics of double charging a hull, forgetting to stick a wad in it, and then double charging the same hull with shot, the resulting shell wouldn't blow up a gun. Without a wad, there's nothing to seal the expanding gases within the bore. And without the seal, there's no pressure. And with no pressure, there's inconsistent ignition. So, all you'll get is a mostly harmless fireball and a bunch of unburned powder in the bore.
In my 40 plus years of reloading shotgun I have made many a mistake but a double load didnot cause this.Shotgun is nothing like rifle as you know. I have seen quite a few barrels blown over the years at a club and 90% were the end result of a wad staying behind from a previous shot, snow or clay in the end of the barrel or in the real old days dropping a red 20ga in by mistake followed by a 12 ga round. Those explosions normally took a few fingers with them however. I still have some red 20ga hulls I kept as a reminder poor picture however.Bottom line check the barrel all the time between shots.So easy in a O/U
Yes sir, many a finger was lost or mangled over those in the day. No wonder they switched to yellow.
