For ptarmigan hunting on the barrens, or grouse/rabbit hunting on the trails I carry the following:

Smartphone (with GPS and topo) with extra battery
Lighter/matches
Firestarter stick
Toilet paper
Leatherman
Buck knife
Choke tubes/wrench
Emergency Poncho
Black electricalTape
Snacks/bars
Hunting license
Small game snare wire
In my vest:
15-20 lead birdshot (sometimes a few extra in waistpack)
5-10 steel birdshot
3 buckshot
Toque
Gloves
Rain coat
Camera
Duck and goose calls
Garbage bags
Eyeglasses/Sunglasses (whichever I'm not wearing)
E-collar remote
small amount of dog food
