If I had a company that dealt with something like this I'd just replace the upper receiver and be done with it. Not every little mystery needs to be solved.
I'm sure they've seen this before in Switzerland - the gun, in some way, is worn out after the 5000+ rounds.

I'd probably bump up that number by a factor of 6 or 7 before saying a SAN is worned out and even then it'll keep on doing.
