Ouch

Out of time is my guess since the barrel flies to one side taking the top strap with it.
Also the manner in which the cylinder is being heavily pushed out under recoil would to me indicate that the bullet hit the forcing cone, sent barrel and part of the top strap flying.
 
Previous squib that put the bullet fully into the forcing cone, second caused this. At lest that would be how I read it.

Check targets? A squib has probably one of the most distinct sounds in the shooting world....even with ear muffs on. Plus the complete lack of recoil would give it away. If you're not checking the barrel after this you almost deserve to have a gun blow up.

Of course, that's assuming this was a squib

I went to the link. It was a rental gun; gun range claims it was an over pressure round from the factory. (500 S & W); I doubt it.
 
I talked to a S&W factory tech. Happens more than reported. Usually with Performance Center products (500 S&W) with high round counts and factory ammo. Probably with reloads too I suppose. I asked him why but he was vague but reassured me if a revolver grenades S&W (Performance Center) will replace it no questions asked. Not sure if this applies to other S&W products? Didn’t ask.

ok, I took another look and at high magnification, the flutes on the cylinder look to be lined up correctly, and yeah a squib load on a .500 would be real obvious - it has been far less so with precision bullseye guns in .32 and .38 match loads. BTW recoil begins with ignition, only perceived due to the slowness of the human nervous system once the bullet has exited the barrel. Only "grenade" effects I've seen from excessively overloaded rounds took the tops off the chambers and the top strap, but left the barrel. This event remains a bit weird to me.
