Nothing new here. It is a known problem with the 500 S&W X frame going back to when they were introduced. Numerous incidents but S&W will not admit to a problem. Do a little research and u will find numerous ranges where this very thing happened with their rentals. One range in LAS has gone through numerous guns as a result but public demand for the big momma keeps it going as its a big revenue generator. You will also find another problem with cracked forcing cones which is more than likely directly related to this problem.
BTW he is very lucky he still has an index finger on his left hand. Note the position of his finger around the front of the trigger guard. Yikes!!
