More likely a double charge.

A bozo came to one of our CAS events with a pair of Ruger Super Blackhawks. He had a VERY loud detonation in one stage. In another stage he had a second such detonation and was asked to unload, show clear and go home. He never came back.
 
If you look closely there was a lot of thumbing going on. Could the shooter have hit the cylinder release?
I was impressed that he kept the revolver pointing downrange......
 
I don't see a hot load blowing off the barrel, unless the gun was already cracked, top strap is still there is it not.
Out of time , it should not fire, but I don't play with 50cal guns.
He is a lucky bugger I would think.
Standard policy when guns blow up is to blame the ammo, in some cases could be correct.
