What happened to having a fourth on the top rail?

Flip-up or AR-style-carrying-handle rear sight on the back of the rail and eyeball it across the top of the shotgun barrel like you're supposed to with scatterguns.

And string up a pulley system to fire all four simultaneously. KA-BOOM!
